Why does this matter?
ABO antibodies are naturally occurring - everyone has antibodies against the A or B antigens they lack. This makes ABO the most critical match.
The D antigen (Rh) is clinically important enough that we match it ahead of time, even without a known antibody. It's highly immunogenic and can cause serious reactions.
Other blood group antibodies only need to be matched if the patient has developed those specific antibodies from prior transfusions or pregnancy.
Understanding Compatibility
Your immune system recognizes blood antigens as "self" or "foreign." When incompatible blood is transfused, antibodies can attack the donor red blood cells, causing serious or life-threatening reactions.
ABO Matching
ABO antibodies exist naturally in your plasma. Getting incompatible ABO blood can cause immediate, severe reactions.
Rh Importance
The D antigen (Rh) is highly immunogenic. Rh-negative patients should receive Rh-negative blood to prevent antibody formation.
Universal Donors
O-negative blood lacks A, B, and D antigens, making it compatible with most patients in emergencies.
